Home & Community Support

 

Whether needed for just a few hours, or overnight, care can take place in your own home, at day-care centers, or at residential or nursing facilities that offer overnight stays. We assist individuals in acquiring, maintaining, and improving self-help, domestic skills, socialization, and adaptive skills necessary to live safely and successfully. Services consist of support in the general areas of self-care, communication, fine and gross motor skills, mobility, personal adjustment, relationship development, socialization, and use of community resources. Habilitation may be provided up to 16 hours a day based on the needs of the individual.

Our trained caregivers have a 24-hour professional support system that ensures the well-being and security of our clients and caregivers. These activities are necessary for individuals to live in the community, to live more independently, or to be more productive and participatory in community life.

Under our Home and Community Support program we offer 3 main services:

 

Companion Care

​

Foremost Comprehensive Health would provide companion care service that is primarily emotional support and companionship for individuals who are generally healthy and who want to remain independent at home. This service can also include a range of non-medical services where patients can maintain their routines and continue to live in an environment that feels secure and comfortable.

​

These services can include but are not limited to the following:

  1. Light housekeeping

  2. Assistance with activities of daily living (ADLs)

  3. Medication reminders

  4.  Planned activities to promote well-being though social and health services

  5. Aid with reading and other hobbies

  6. Arrange and provide reminders for appointments

  7. Care for houseplants and pets.

 

Though companion care focuses on those individuals in their homes, it can be provided to individuals in nursing homes and assisted living facilities.

Respite Care

​

We provide temporary relief for a primary caregiver, enabling you to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member. This service can be arranged for just an afternoon or an extended period to accommodate vacations, moves, and/or other personal life adjustments. This form of care can be provided at home, in a healthcare facility, or at a patient day center. Respite care is an essential part of the overall support that families may need to keep their family member with a disability safe and cared for.

 

Foremost Comprehensive health provides Respite care per the Department of Public Welfare title 55 code chapter 52.14, as follow:

​

  • Respite services are provided to support individuals on a short-term basis due to the absence or need for relief of those persons normally providing to the consumer.

  •  Respite Services are provided to individuals in their own home, or the home of a relative, friend, or other families, and are provided in quarter-hour units (15 minutes increment).

  • A relative or family member may provide respite services as long as the relative or family member is not a legal guardian, power of attorney, or reside in the home.

  • The respite worker must be able to demonstrate the capability to perform health maintenance activities specified in the participant’s individual service plan (ISP) or receive the necessary training

  • In-home Respite services cannot be provided simultaneously with Personal Assistance Services.

  • The frequency and duration of this service are based upon the participant’s needs as identified and documented in the participant’s individual service plan (ISP).

 

Homemaker services

​

Everyone enjoys a clean home, and nothing is more important for those dealing with mental or physical challenges than a clean and tidy home! Having a well-maintained home improves mobility, enhances mood, and can help keep someone healthy. We offer Homemaker Services to help create a safe, clean, and comfortable living environment so loved ones can continue living independently. Services range from basic cleaning, performing laundry, and a light dusting to deep cleaning, maintenance and homemaker/home management services.
